vildagliptin is another orally administered dipeptidyl peptidase-IV (DPP-IV) inhibitor after sitagliptin. Vildagliptin is marketed in the European Union (EU) by Novartis Pharmaceuticals in 2008 for the treatment of type 2 diabetes mellitus. Diabetes mellitus is a chronic metabolic disease with an increasing prevalence rate year by year. Type 2 diabetes mellitus is a complex disease with multi-gene genetic factors and environmental factors acting together. Dipeptidyl peptidase IV(DPP-IV)Chemicalbook inhibitors are a new class of anti-diabetic drugs, which play a hypoglycemic role by inducing and promoting in-sulin biosynthesis and secretion, inhibiting β cell apoptosis, inhibiting hyperglycemic secretion, reducing food intake and other mechanisms. Moreover, it can reverse the deterioration of pancreatic function in patients with diabetes while controlling blood sugar, which shows a good application prospect. Vigliptin is a representative drug of dipeptidyl peptidase inhibitors. In clinical studies, whether used alone or in combination with metformin and in-sulin, it has shown good anti-diabetic effect and tolerance.

 

Vogliptin is a selective, competitive and reversible inhibitor of DPP24. Glucose-dependent in-sulin stimulating polypeptide (GIP) and glucagon-like polypeptide 21(GLP21) are important hormones for maintaining glucose concentration in the body, and both have in-sulin stimulating effects. In type 2 diabetes mellitus, the in-sulin promoting effect of GIP is impaired, and only GLP21 can play the role of promoting the secretion of islet hormone, which promotes the secretion of in-sulin by acting on receptors on the cell membrane of islet β. GLP21 also inhibits glucagon secretion to Chemicalbook and gastric emptying to increase satiety (suppressing appetite). DPP24 binds to proteins in many tissues, such as the brush edges of the membrane of the kidney, liver, small intestine, pancreatic ducts, lymphocytes, and endothelial cells, and rapidly inactivates GLP21 by hydrolyzing its N-terminal alanine 2. This product inhibits the activity of this enzyme by combining with DPP24 to form DPP24 complex, which increases the concentration of GLP21, promotes the production of in-sulin by the pancreatic β cells, and reduces the concentration of glucagon, thus lowering blood sugar. There was no significant effect on body weight.

 

1. The most common adverse reactions were headache, nasopharyngitis, cough, constipation, vertigo and hyperhidrosis. Very few patients with hypertension appeared, but the correlation with the drug is not certain. Nearly all of the studies confirmed that the incidence of hypoglycemia with Vogliptin was similar to that with Chemicalbook placebo.
2. A controlled trial found that, compared with thiazolidinediones, the incidence of headache, rash and other adverse reactions was similar in patients taking Vogliptin, but the incidence of treatment termination due to adverse reactions and serious adverse reactions was lower in the Vogliptin group.
